Insurance companies that have previously not included the services of an Athletic Therapist in their extended health policies are responding to the increasing demand of their beneficiaries for these services. This response has been steady, if not rapid, and is encouraging for patients who find Athletic Therapy invaluable to their health.
We encourage you to check with your Human Resources Department and/or your policy provider to determine whether your policy includes the services of an Athletic Therapist.
If you discover that these services are not included, or are included at a lower level than other health professionals, we urge you to download the forms located on this page and send them to the appropriate parties. Shouldn't you have the option to choose the therapy that works best for you?

Hockey Therapy
Need help paying for your Athletic Therapy? If you're a carded hockey player, coach, or official with no other coverage options, these are amazing plans!
Minor hockey participants, including female teams, must use the
BCAHA Mutual Aid Injury Report Form. A physician's referral is not required. Claims must be received within 90 days.
Junior and Senior level players, and officials of all levels, must use the
CHA Injury Report Form. A physician's referral is required. Claims must be received within 90 days.

Sport Accident Insurance
Many other Provincial Sports Organizations (PSOs) also have supplemental Sport Accident Insurance policies in place to assist their members. If part of the fees you pay to play your sport are sent to a PSO as membership dues, you are likely covered under that governing body's plan.
Here in BC, many PSOs have policies through All Sport Insurance. These plans also demand that you exhaust any and all other coverage for Athletic Therapy that might be available before starting a claim. If you have done so, or have no coverage for Athletic Therapy, contact your PSO to determine whether they have such a policy in place. Eligible parties must fill out the All Sport Athletic Accident Claim Form.
A physician's referral is required. Notice of claim must be received within 30 days.
